New Jersey Hospital Performance Report meaures New Jersey hospitals on their performance in providing quality treatment to their patients with heart attack, heart failure, pneumonia, and surgical care infection prevention, based on national standards.

Vital Statistics:
The Fair Lawn Registrar has Vital Records for births, deaths, marriages, civil unions and domestic partnerships that occurred in Fair Lawn.

Certified copies of certificates of births, deaths, marriages, civil unions and domestic partnerships, occurring in Fair Lawn, can be obtained via written request at the rate of $10.00 per copy. Forms related to vital records can be found on the Documents page under the Health Department heading.

Marriage License:
Marriage license applications are taken from 9:00 AM to 3:30 PM weekdays by appointment. A 72 hour waiting period is required from application to issuance. The bride or groom must be a resident of Fair Lawn. There is a $28.00 fee. Applicants are encouraged to call the Registrar for details 201-794-5327. Fish Consumption Advisory:  The NJ DEP (NJ Department of Environmental Protection) and the NJ DHSS (NJ Department of Health & Senior Services) provide advice on consuming species of fish in which high levels of dioxin, PCBs and mercury have been found. Fish Advisories can be found at http://www.fishsmarteatsmartNJ.org

Loaning of Medical Aids:
The Fair Lawn Health Department in conjunction with the Fair Lawn Volunteer Ambulance loans wheelchairs, crutches, walkers and canes to Fair Lawn residents free of charge for 60 day increments. Donations of wheelchairs, aluminum crutches, walkers and canes are accepted by the Department. All equipment is loaned out during regular business hours of the Health Department only. Call 201-794-5327 for more information. Call before coming to pick up the equipment, there may be a 24 hour advanced notice required.

Community Noise: Noise from domestic power tools, lawn mowers and leaf blowers are impacting on the community. Be advised that these items can only be operated between 8 AM and 8 PM weekdays and 8 AM to 9 PM on weekends and legal holidays, provided they produce less than 85 decibels. 

Outdoor Pesticide Use: Any outdoor spraying requires notification of adjacent properties within a 100 feet radius of the spraying area. When hiring a pesticide applicator, make sure they are properly licensed by the town and state. For specifics, check the Borough ordinance section of the website. The pesticide ordinance is chapter 166.  For non-compliance with this ordinance, call the Health Department at 201-794-5327.

Residents who suspect pesticide misapplication should call DEP's Bureau of Pesticide Compliance at 609-984-6568 (during off hours, ad suspected environmental violations should be reported to DEP's 24 hour hotline at 1-877-927-6337). For a pesticide health emergency, call the NJ Poison Information System at 1-800-222-1222. Through the Federal EPS'a website at www.epa.gov, individuals can access a Citizen's Guide to Pest Control and Pesticide Safety. Local Rutgers County Extension offices also provide access to good sources of specific pest control information.

Low Cost Blood Screening / PSA: Event will be held on June 17, 2011, for Fair Lawn residents at the Fair Lawn Community Center. Register in person at the Health Department in Room 112 at Borough Hall. Appointments can be scheduled at the Health Department and payment is required at the time of registration. Fees are set by the laboratory each year and usually is in the $20-$30 range for 23 different elements in blood including: total cholesterol, glucose for diabetics, triglycerides, complete blood count, and cardiac risk profile. The prostate cancer blood test for men is an additional fee.

Free Blood Pressure Screening: Last Monday of the month in Borough Hall, Room B-5, from 1:30 PM to 3:30 PM except May and December. Usually the second Friday of the month from 9:30 AM to 11:30 at the Senior Center.

Free Health Consultations: Including hypertension check, with a public health nurse are provided on Tuesdays from 9:30 am to 11:30 am in room B-5 of Borough Hall. During consultations, the public health nurse will take a blood pressure, perform a risk assessment and discuss health concerns with participants.

Free Child Health Clinic: First and Fourth Thursday afternoons of the month from 2 pm to 4 pm. This program provides well baby checkups that include developmental assessments, certain free immunizations and health education for children for children under age 6. The program will also see school aged children in non-compliance for the immunizations. This program is intended for children without health insurance or whose health insurance does not cover the cost of immunizations.  Appointments are mandatory and may be made by calling 794-5331.

NJ Family Care Packets: NJ FamilyCare is affordable health care coverage for uninsured kids, parents, couples and even single adults. NJ FamilyCare is not a welfare program. It is low cost health coverage through a variety of recognized Health Maintenance Organizations. To find out if you are eligible, to enroll, or for more information, call 1-800-701-0710 (multilingual operators are available, or visit their web site at www.njfamilycare.org, or call the Bergen County Board of Social Services at 201-368-4200. 

Child Blood Lead Screenings: Low cost child blood lead screenings are available through the nursing division. This program is intended for children under 6 years of age. The $10 fee for this program is paid directly to the laboratory for their analysis. This service is available to all regardless of income. To arrange an appointment with the public health nurse, call 201-794-5331. 

Free Cervical, Breast and Colorectal Cancer Screenings: Available for women 40 years of age or over, who must meet income requirements or other guidelines.  Call 201-634-2660 for more information. The YWCA also has a breast cancer education and screening program for women over 40 years of age and no medical insurance. Free Prostate and Colorectal Cancer screening: Available to men aged 40 and over who meet income and other guidelines. Call 201-634-2660 for more information. 

Food Handler Classes:   Food handlers employed in Fair Lawn are required to attend a food handlers class in food safety at least once every 5 years.  Several food handler classes are offered  throughout the year both in the daytime and the evening in the Municipal Building Council Chambers at 8-01 Fair Lawn Ave.  See Food Handler’s Course 2011 Listing  (link)  Food handlers employed in other towns may register for a Fair Lawn class for a $25 fee, but should check first with the Health Department in the town where employed.  See attached Registration Forms (day class / night class) - note December 7th class changed to December 14th.

Smoke Free Restaurants: As of April 15, 2006, all restaurants as well as all other workplaces in New Jersey, except Atlantic City Casinos, are required by law to be smoke-free environments.

If you are exposed to Environmental Tobacco Smoke (ETS) in Fair Lawn, whether it be at work or in a restaurant and wish to register a complaint, call 201-794-5327 to speak with the environmental health specialist. Health officials will enforce existing laws where they apply and where laws are insufficient,  assistance to resolve the problem will be provided.

Stop Smoking Web Site: Smokers seeking assistance to stop their tobacco habit are urged to visit www.nj.quitnet.com , for 24 hour information, or call the toll free #: 1-866-NJ-STOPS (1-866-657-8677) from 8 AM - 8 PM Monday to Friday and 11 AM - 5 PM on Saturdays. 

Environmental Commission: Meets the first Wednesday of the month at 7:30 PM in room B-5 at Borough Hall.

Pneumonia Immunizations: Continue to be available year round to residents ages 18 and over who have never had the immunization. The fee for this service is $25. Call the public health nurse at 201-794-5331 to arrange an appointment or if you have any other questions. 

Alliance for Substance Abuse Prevention: Board meets monthly, on the 3rd Wednesday of every month, except February, July and August. All residents are invited.  Visit the FLASAP web page for meeting dates/times, meeting minutes, mission statement and more.

West Nile Virus Surveillance: West Nile Virus surveillance includes compiling ill and dead bird reports, testing mosquitoes and crows, which are dead less than a few hours. Reports received from residents are reported to the NJ Department of Health. Since the virus is spread by the bite of an infective mosquito, the Bergen County Mosquito Division collects mosquitoes for testing. They also regularly check select known water sources and treat, when necessary, to prevent mosquito breeding. Residents are asked to assist by checking their own properties for any items that hold water, such as buckets, tires, toys, trash cans, etc. and either discard them or turn them over so they do not collect water. Anyone wishing to collect rain water for watering gardens should cover the barrel with fine mesh screens or use salad oil to cover the top of the water. To report ill or dead birds for WNV, call 201-794-5327. West Nile Virus season is May through October.

Emergency Response:  In the event of a large scale disease outbreak or disaster, municipalities will need additional medical professionals than the present staff.  Medical professional volunteers are needed.  To find  out more about the Medical ReserveCorp, visit  http://www.bergenhealth.org/mrc .
